I am contacting you because I am extremely concerned about my child's future education. I believe that my child is suffering from a specific learning difficulty (dyslexia) and consequently I am referring her/him for a
statutory assessment under the 1989 Education Act (a formal request from a parent, page 42 Code of Practice) so that the necessary help can be made quickly available.

* Since beginning school my child has become very withdrawn, frustrated when trying to do her/his homework, lacks in confidence and has very low self esteem.  Therefore, you should treat her/his need for an assessment as a matter of urgency.

My daughter/son is being prevented from receiving the help needed because a statutory assessment has yet to be carried out.


The Code of Practice says that the LEA have 6 weeks to state if they
will or will not make a statutory assessment and I look forward to hearing from you within this strict time period.
